Care Management Medical Assistant


This is a Full-time position in Saint-Petersburg, FL posted July 29, 2020.


The Care Management Medical Assistant assists and supports the Care Management Team with daily operational tasks, data input and collection, clerical and administrative duties.


  • Consistently exhibits behavior and communication skills that demonstrate commitment to superior customer service, including quality and care and concern with each and every internal and external customer.
  • Represents the Company in a professional manner, following all Company policies and procedures.
  • Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
  • Completes administrative functions associated with Care Management tasks
  • Assists patients with external resources at the direction of the Care Management nurse
  • Utilizes various data sources and enters data in various programs and spreadsheets 
  • Maintains call logs, excel spreadsheets, and data findings electronically
  • Compiles, organizes, and mails out materials to internal and external customers 
  • Performs general clerical duties such as typing correspondence, creating or updating spreadsheets, photocopying, faxing, and compiling data and other materials or other duties as required
  • Obtains necessary patient information from clinical centers in order to assist with discharge process
  • Obtains medical records and faxes to Scanning department for entry into EHR
  • Enters referral requests
  • Assists with scheduling post discharge follow up appointments with PCP when requested
  • Other duties as assigned


  • High school graduate, GED or equivalent
  • Medical Assistant Diploma


  • Experience in a health care related setting


  • Ability to communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner using excellent customer service skills
  • Possess high degree of accuracy, efficiency, and dependability
  • Ability to perform multiple tasks and possess strong organization skills
  • Ability to follow written and oral instructions and to work with general guidance
  • Knowledge of HIPAA regulations
  • Must have consistent, punctual, and reliable attendance
  • Ability to travel may be required
  • Ability to read, write, speak and understand the English language fluently
  • Possess excellent communication and organizational skills with the ability to multi-task, set priorities, and meet deadlines
  • Results and goal-oriented with a philosophy for quality improvement
  • Must be computer literate and possess the ability to master different medical record software programs
  • Demonstrate exceptional telephone skills
  • Demonstrated proficiency in Microsoft Office, Email and general computer functions
  • Proficient in Excel spreadsheets
  • Strong communications skills
