Vancouver Coastal Health is looking for a Registered Nurse with experience in diabetes education to join the Healthy Living Program at Pacific Spirit Community Health Centre. Apply today to join our team!

 

As a Chronic Disease Prevention & Management Nurse with Vancouver Coastal Health, you will:

  • Utilize knowledge of diabetes nursing practice and care to design, develop, implement educational and self-management sessions integrating Chinese cultural values using established Western diabetes management programs for clients/families/caregivers to achieve optimal diabetes management.
  • As a member of the interdisciplinary Healthy Living Program team, participates in the development, implementation, delivery and evaluation of screening, education, early intervention and disease prevention and management programs.

QUALIFICATIONS:

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nursing Professionals (BCCNP).
  • Three (3) years recent clinical experience in a diabetes education setting or an equivalent combination of education, training and experience.
  • Canadian Diabetes Educator Certification.
  • Language Fluency consistent with the patient population need of the program.
  • Valid BC Driver’s Licence as local area travel may require the use of a vehicle.
  • Proficiency in Skype, Zoom and Video Education.
  • Proficiency in Cantonese, Mandarin.

Knowledge & Abilities

  • Comprehensive knowledge of nursing theory and practice within a client and family centred model of care.
  • Comprehensive knowledge of the BCCNP’s standards of practice for registered nurses.
  • Comprehensive knowledge of evidence based nursing practice related to diabetes (prevention/management) and diabetes education.
  • Comprehensive knowledge of health promotion & population health strategies and frameworks/models.
  • Comprehensive knowledge of chronic disease management and self-management support.
  • Working knowledge of community resources.
  • Comprehensive knowledge of other health care disciplines and their role in patient care.
  • Comprehensive knowledge of cultural diversity and its relation to healthy lifestyle behaviors.
  • Broad knowledge of research process and methodology.
  • Demonstrated ability to provide leadership and work direction.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Demonstrated ability to communicate orally and in writing.
  • Demonstrated ability to develop and present educational programs and materials.
  • Demonstrated ability to communicate with, and deal effectively with, co-workers, physicians, other health care staff, patients and their families, and outside agencies.
  • Demonstrated ability to adjust to new or unexpected events.
  • Demonstrated ability to plan, organize and prioritize work.
  • Demonstrated ability to deal with and/or guide others in resolution of conflict issues.
  • Demonstrated skill in the use of medical equipment and supplies appropriate to Diabetes.
  • Demonstrated skill in techniques appropriate to Diabetes Education.
  • Demonstrated skill in CPR techniques.
  • Demonstrated physical ability to perform the duties of the position.
  • Basic computer literacy to operate a computerized patient care information system.
  • Demonstrated ability to access computerized multi-media resources for diabetes education.
  • Demonstrated ability to organize, analyse and problem solve.
  • Ability to operate related equipment.
