Nurse Care Manager

Meadows Retirement Community

This is a Contract position in Chenoa, IL posted June 12, 2020.

This is a new position, therefore, we have a 1st shift and 2nd shift position available.Job Summary:Nurse Care Manager is a leadership position assisting the Director of Nursing (DON) in management of nursing – under the Scope of the State of Illinois Nurse Practice Act for Registered Nurse licensure. This is a working leadership position. The position coordinates resource utilization, timely and appropriate interventions, and interdisciplinary communication to enhance resident and family satisfaction, adherence to Meadows’ clinical systems and regulatory compliance for the center. The Nurse Care Manager is responsible for managing the nursing department in the absence of the DON.Qualifications:A minimum of two years experience as a RN in a long term care facility with one year in a supervisory capacity.Education:Graduate of an accredited nursing school. RN licensed in good standing in the State of Illinois. Bachelor’s Degree in Nursing preferred.Work Experience:A minimum of two years experience as a RN in a long term care facility with a minimum of one year in a supervisory capacity.Knowledge, Skills and Requirements:* Excellent interpersonal and customer service skills.* Ability to communicate clearly in oral and written form.* Must be creative, flexible, and understanding;* Comprehensive understanding of medical records guidelines and standards.* Ability to convey a strong support for Meadows Mennonite Retirement Community’s ministry to staff, community and supporting churches and to support the mission statement.* Ability to relate well with the elderly, family members and key decision-makers.* Excellent organizational skills.* Strong computer skills; must have working knowledge of Microsoft operating system, including Word, Excel, and Outlook; experience with database management.* Ability to write reports and business correspondence.* Must have good vision with or without glasses/contacts in order to perform weekly data entry and review correspondence before being sent out to the public and/or Meadows Mennonite Retirement Community personnel.* Must be able to satisfactorily complete pre-employment fitness testing.Essential Functions:* Establishes a compassionate environment by providing emotional, psychological, and spiritual support to residents, friends, families, and staff.* Maintains current knowledge of federal and state regulations including professional standards.* Acts as a clinical expert in the care and management of the resident.* Acts as a role model in the assessment, planning, delivery, and evaluation of standards of excellence based care, ensuring excellent resident care is delivered using a holistic based model including resident and family involvement.* Demonstrates sound level of clinical judgment and decision making in implementing and/or modifying interventions.* Creates opportunity within the clinical setting to share clinical expertise through teaching and coaching staff.* Maintains clinical credibility and recognition within the multidisciplinary team.* Participates in disciplinary issues in accordance with policy.* Operates computer software to enter and retrieve data.* Maintains a cooperative relationship among health care teams by communicating information.* Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications; establishing personal networks; participating in professional societies.* Accomplishes organization goals by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.* Oversees in-service education to meet the needs of the staff and regulatory requirements.* Attends in-services, meetings, conferences, and workshops as requested to keep abreast of current trends in gerontological practices.* Serves as a member of committees as required or requested.* Represents Meadows Mennonite Retirement Community as appropriate in community, church, or health related agencies and committees.* Participates in the “on call” rotation for nursing staffing and clinical issues after business hours. Fills in as nurse as needed.* Responsible for assisting with daily staffing as needed.* Perform other related duties as requested.* versees, directs, and coordinates nursing staff to provide proper resident care consistent with nursing practices.* Oversees the provision of appropriate medications, treatments, and general nursing services according to the resident’s care plan and physician directed orders.* Abides by all policies and procedures of Meadows Mennonite Retirement Community.* Other job related functions as assigned.
