This is a Full-time position in Newport News, VA posted June 10, 2020.

Job Description Under the general supervision of the Office Manager the Health Coach works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team the Complex Case Managers and other health care and community services.

Key duties include a strong focus towards preventive health care as it relates to annual wellness visits and closing care gaps.

This practice based health coach must be able to demonstrate effective motivational interviewing techniques as well as demonstrate critical thinking problem solving and decision making skills to positively impact the optimal health of a patient panel.

The area is very unique with a large number of military bases and several large colleges and universities as well as other large healthcare systems.

The attractions are endless with so many historical sites to visit and a long list of museums and theaters.

Educaiton: LPN Education required to meet licensure requirements.

Experience: Two years of LPN experience preferred in an doctor’s office setting or comparable experience (i.e.

medical assistant, corpsman would receive 2/1 credit for previous experience).

Certification/Liencsure: Licensed Practical Nurse or board eligible LPN by the State of Virginia required.

CPR required.
