Social Worker – MSW – Community Mental Health

This is a Contract position in Richmond Hill, ON posted June 29, 2020.

Social workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individual, family group and couples. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education.

St. Michael’s STEPS for Youth Program is designed to provide early intervention services for individuals aged 14-35 who are experiencing signs and symptoms of a first episode of psychosis. Assessment, treatment and community-based case management services are provided by an interdisciplinary mental health team for up to three years. 

The successful candidate for this position must demonstrate the ability to:

  • Work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care
  • Actively contribute to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community
  • Identify and address educational needs for patients and their families
  • Demonstrate a commitment to ongoing professional development in order to continue to provide optimal patient care
  • Commit to, and participates in, ongoing quality improvement in the organization
  • Provide patient care in accordance with all professional and organizational standards
  • Demonstrate an understanding of the social determinants of health and knowledge of community resources
  • Utilize assertive outreach strategies to deliver services and supports in the client’s home or in the community
  • Demonstrate excellent knowledge of early intervention in psychosis principles and approaches
  • Possess skills in crisis intervention and other evidence- informed practice in therapeutic modalities (CBT, DBT, solution focused interventions, mindfulness).
  • Utilize excellent problem-solving skills in establishing therapeutic relationships with hard-to-engage clients and support individuals in their goals


  • Must have a Master’s Degree in Social Work (MSW) or Equivalent
  • Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas
  • 2-4 years of experience as a Social Worker preferred
  • Must be willing to conduct home visits with patients in the mid to down town Toronto areas
  • Must have experience working with individuals who experience mental health and addiction and with people living with chronic conditions
  • Comfortable with community engagement and outreach
  • Eligible to engage in student education with academic partners
  • Current CPR certification
  • Excellent communication, critical thinking and interpersonal skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care

Must be willing to work evenings and/or weekends as directed by the Program.
